Will you recognise your heart attack? – New educational material for patients and their families.

The Heart Foundation is launching a program to educate patients with coronary heart disease (CHD) and their families about heart attack warning signs, what to do and why it’s important to act quickly if they have them. You are in the ideal position to help.

We have developed a "Will you recognise your heart attack?" action plan and fact sheet (see below). You can help to reduce patient delay in calling Triple Zero (000) by giving the action plan and fact sheet, along with education and counselling, to patients before discharge from hospital following a coronary event, and during cardiac rehabilitation.

Access the new Patient Action Plan, Health Professional and Patient Fact Sheet;

Encourage your patients to enjoy the benefits of walking, as it's Spring!

How about referring them to Heart Foundation Walking? We have Australia’s largest network of free community-based walking groups led by volunteer Walk Organisers.

Joining or starting a Heart Foundation Walking group in your area or workplace is a great way to get active and meet people. Every walking group is different - they vary in the number of members, and levels of difficulty and meet at a variety of times and days of the week. Everyone is encouraged to walk at their own pace.

Over the last 10 years, more than 15,000 Australians have enjoyed the benefits of being part of a Heart Foundation walking group and living a more active lifestyle.

To find out about a Heart Foundation Walking group near you or to start your own group call 1300 36 27 87 (local call cost) or visit our web site;

Now available on our website is our Coronary Angiography fact sheet for you to download and use with your patients. The fact sheet explains what a coronary angiogram is, when it is required, the risks involved, the preparation that takes place in the hospital and outlines the actual procedure.

Additionally our Coronary Angioplasty and Stent Implantation brochure has been updated to be a comprehensive resource, including information on angiography. This is available in brochure format.

Import our GPMP for CHD template into your Genie software.

A GP Management Plan (GPMP) for coronary heart disease (CHD) has been developed for Genie software and it is now available to download from the Heart Foundation website.

Our General Practice Management Plan (GPMP) is designed to support general practice by using the chronic disease management (CDM) Medicare Benefits Schedule (MBS) items in implementing best practice interventions. The GPMP is based on the Heart Foundation’s "Reducing risk in heart disease 2008" guidelines to improve ongoing management and secondary prevention of patients with CHD.

For instructions on importing the template, go to:

Gill and Donna will complete this challenge in memory of Gill’s youngest son Mark who died due to heart disease in 1988 at the age of just 15.
